HMGCR antibodies


Suspicion of immune-mediated necrotizing myopathy (IMNM). Anti-HMCGR is strongly associated with previous statin therapy.




The test result is given as negative or positive with a value.


Antibodies against HMG-CoA reductase (HMGCR) are associated with necrotizing autoimmune-myopathy and can be detected in up to 25% of these patients. Often it is seen greatly increased levels of creatine kinase (CK) in blood. Pronounced proximal muscle weakness is typical while lung involvement is less common. Almost half of the cases have previous treatment with statins. The level of anti-HMGCR is well correlated to the muscle strength and level of creatine kinase (CK) in blood. Anti-HMGCR-associated myositis often respond well to immunosuppressive therapy. Anti-HMGCR is usually not detected in healthy subjects or in subjects with other inflammatory and autoimmune diseases. Concurrent occurrence of autoantibodies against SRP (signal recogition particle) may occur in occasional cases and is prognostically unfavorable.

HMG-CoA reductase is an enzyme that catalyzes the reaction between HMG-CoA and mevalonic acid in the new synthesis of cholesterol. The discovery of HMG-CoA reductase inhibitors, also known as statins, was a breakthrough in the treatment of hypercholesterolemia. Statins are used to block the complex biosynthesis of cholesterol.
